I was a patient at Fountain Valley Post Acute for seven weeks. I could provide a detailed review for everyone that works there and every person there would get a very positive review however, that would take much more time than I have. I simply want to say thanks to everyone there that helped me through what was the largest medical crisis that I ever had to deal in my life. Also, they provided an empathetic atmosphere which in my opinion is the most critical component that I required. Thanks Fountain Valley Post Acute, You saved my life and I’m greatful that my hospital sent me there after my surgery! William N.

I’m very confused on all the amazing reviews on this facility. As a physician and surgeon myself, I was appalled at the care that my grandfather was receiving here. A skilled nursing facility encompasses quality around the clock care for patients in order to get them to their baseline health. For my grandfather who suffered from a stroke and subdural hematoma with a high aspiration risk, not only were there delays in taking his blood pressure to keep it under strict control, there was also significant delay in administering his medications. Nursing took a long time to address his secretions when we called for help- aspiration is a leading cause of mortality/death for such patients by the way. I watched as the nursing assistant wore the same gloves to help wipe the bed, then his bottom, then his head where he had just had a surgery and drain placed. I can’t even begin to describe how awful the cleanliness is here and how difficult it was to get him the care the we trusted them to give. Again, as a physician I do my best to give other healthcare providers my trust that they would care for my loved ones the way that they should be. But after being here for several days, my grandfather again declined and had to be sent back to the hospital. Furthermore, there were no physicians or medical providers who SHOULD HAVE BEEN AVAILABLE to evaluate him when his mental status deteriorated. And yet, they said “we don’t have any doctors or providers to evaluate him.” I will absolutely not recommend this place to anyone.

My mom stayed for a brief duration following a stroke. While no one wants to be in a rehab facility, I must compliment the staff. Samar and Javier, as well as her social worker and doctor, were kind, patient, and very warm. I don’t think my mom loved her meals, but the staff went above and beyond. Truly above and beyond. Her physical therapist was encouraging and patient. The facility is clean and well organized. No one likes spending time in a rehabilitation facility and recovery can be hard, but I appreciate the kindness of everyone there.

Samar is the sweetest person to greet the clients and patients. I am always happy to see her and she is so positive and kind. She is one of a kind and a huge asset to the facility. The nursing staff was also kind and helpful. I really enjoyed my stay. However, the food wasn’t very good but the staff was amazing!

So far, the worst. Cannot even get a case mngr to call back after my father was admitted 5 days ago. Many many problems. I will amend this as I get more info. But, beware, you might want to keep looking or find yourself a patient advocate if your loved one is admitted here.

Samara and Gizelle (sorry if I spelled wrong) Were so helpful and friendly. The facility looked clean and friendly. I feel comfortable that my dear friend is being cared for.

FV Post Acute provided my family member excellent care. Jessica in Social Services was so supportive and helped to make sure the care was well coordinated. The nursing and CNAs were responsive and available to provide updates. The facility is clean and fresh!

My husband was admitted for atleast 12 days @ Fountain Valley post acute. The nurses staff are very professional & accommodating to the patients needs. The front desk Samar is very accommodating & friendly.. PT May is very hands -on motivates my husband to move farther. CNA Brandy is very hard working & compassionate . Thank you for all your patience & hard work.

I am writing to share my positive experience regarding the care for my 81 y/o dad provided at Fountain Valley Post Acute. I am very impressed with both the facilities and the staff where he is currently being cared for. The facility is well-maintained and provides a comfortable environment for recovery. Additionally, the staff members are exceptionally professional, attentive, and kind, ensuring that all his needs are met with high-quality care. The nursing staff is exceptional! That goes from the CNA’ to the RN’s. I trully was apprehensive to go to a post acute facility. Thankfully my daughter did her research and I was not disappointed. The care I received was exceptional. The physical therapy department should be given special recognition. They genuinely care and challenge you to increase your mobility. The only downside side of my experience was the food. Understanding wasn’t there for the dining experience, yet would have been nice to be able to eat some meals with flavor. Thank goodness my family were able to bring me meals at times.

Samar is a great executive and a really fine person to have assisting me when I was checking My Wife😇 in at your facility. Don’t lose her, she’ll make you alot of money. Its not easy to find great perople like her! She gets 5 stars! There were so many fine people here that it would be hard to remember all of their names, but it is an excellent place to be when you’re in need after surgery or a medical problems. Thank you to all the therapist, CNA’s, maintence staff, and nurses here. They have been wonderful. Regards Lauren and Ray Conrad!

This rating as follows: Meh – 2 stars @ leadership could be improved, particularly around sharing and updating new policies: after the fact notice of new policies updates, I was not giving a chance to take my loved ones belongings from their space and the pictures in frames that my parents collected “walked out” of Fountain Valley Post Acute even though we were told that all items we put in a safe place for us to collect. However, not the case, WE LOST ALL the PICTURES! Would not recommend this place for loved ones. 3-4 starts @ the nurses and the front desk are personable and try to help whenever they’d

On May 11th my wife will be discharging from Fountain Valley Post Acute Rehabilitation Center. It is truly a joyful day for our family as we celebrate her recovery and return home. During the three months of her stay, I visited often to care for her. FVPA has proven to be a warm, welcoming, and supportive place for both patients and their families. Everyone here is incredibly friendly, kind, and dedicated, working wholeheartedly to meet patients’ needs and provide the best possible care. It is rare to find such a professional yet genuinely compassionate environment. I would especially like to recognize Samar, Chau Tran, Wei Viyan, Rocio, Victoria Joves, Tiffany, and Jannette. It has been an honor to meet each of them. Their bright, warm smiles—especially in the mornings—made every visit comforting and reassuring, truly like being at home. Samar, in particular, always greeted my family and me with such kindness and professionalism, which meant a great deal to us. I agree with all the others, Samar is the best! Caring, knowledgeable and sweet the entire time of my stay after full shoulder replacement. I’ve got back surgery coming up shortly and am looking forward to staying for a few weeks at Post Acute Care again. The nursing staff are so professional as well, I would recommend aftercare here for anyone I know. Can’t wait to see that smile Samar and the rest of the staff as well. Thanks, John. **UPDATE** Back surgery was a success and I was transfered to Fountain Valley Post Acute Care after a week. My insurance company only authorized an 8 day stay for this procedure which was not enough I feel and leaving while still experiencing pain levels of 8 to 10 after daily exercisesis not good i feel. That said, this was another great experience as soon as I arrived I was greeted at the entrance with a familiar smile from Samar! I instantly started feeling better, was transfered upstairs to basically the same staff I had on my prior visit. The med nurses and CNA’s were just as professional and caring as my first visit. PT and OT Mark, Hannah my dream team! Rosa and Mary I think are the other 2 on the team that are just as wonderful, i just hate getting names wrong. Anyhow I’m in home recovery now, I was a little concerned doing it alone, but seems to be working out so far. Definitely due to getting great care at Fountain Valley Post Acute Care made the difference before coming home. Leaving was hard this time after making these bonds, but ill be back for an update visit. Thank you all again so very much for the great care you’ve provided and Samar as always for the love and care you exhibit towards each and everyone that enters the doors there, it’s effected my recovery greatly and it should be recognized by management there at the center. This is my mom’s second stay at FVPA. The staff is attentive, caring and on top of mom’s needs so that I just get a brief text reminding me to bring extra clothing or treats…in short, they make me look like a doting son (hero). Life or health challenges are overwhelming without a good support team. I truly appreciate so many people there that a list would only prove that I’m absent-minded…but here goes: Shout out to Samar and Chris in reception, Bernadette on the 1st floor, Jamar and Abby on the 3rd floor, and all the caring CNAs (Elva, Sandy, Brandy, Raj, Elexus, and Karen.) This review covers all the folks I can remember since October 2025. I apologize in advance if I forgot anyone. However, in our busy world, full of distractions and details, true heroes often go unnoticed. FVPA staff always took the time to help me make the best decisions for my mom’s care…it is just that simple. Thank you FVPA.

We are grateful for the services from dedicated nursing staff, therapists (physical, occupational, speech), activities director, social workers, and dietary staff who tirelessly work at FV Post Acute Care. Thank you for the continued compassion and care you give to my mom, and most of all for showing kindness, patience, and empathy towards our mom’s complex condition. We found the ICT/IDT meeting productive with the staff and they listened to our concerns and promptly address them. Highly recommended SNF stay towards recovery.

Samar was very kinda, helpful and a professional expert. She is great at what she does for everyone. She is perfect at what she does here. Spring has helped me in getting the right food and preferences. The social workers have all be helpful and helped me with what I needed. Overall the facility was a good experience while I was here.

EXCELLENT care!!! My brother’s been here for some weeks & ALL of the Nursing staff (morning, noon and night!…) are fantastic. Attentive, caring & patient (esp needed w/ my grumpy brother!…) and Social Worker Jessica has gone ‘above & beyond’ helping w/ his case.

All of the employees are so courteous and attentive! These employees include professionals with various specialties who regularly check on the patients to ensure that all clients have everything they need to ensure optimal health care. The team at this facility are extremely supportive and well deserve the highest of ratings. The meals are nutritious, carefully planned, and provide plenty of variety.
